Learn how to complete a living will and designate a healthcare power of attorney with Sara Chambers, BSN, RN, CHPN, Community Educator with Hospice of Northwest Ohio. After the presentation, participants will have the opportunity to set up a one-on-one appointment with a certified facilitator to complete these forms – for free!

The history of advance directives
How to complete a LW/HC-POA
Surrogate decision-maker rules for each state
Question and Answer
